Nurse Practitioners

Nurse Practitioners (NP) are nurses with advanced university education who works both independently and in collaboration with other health professionals to provide patients with quality health care services. NPs can diagnose illness and injuries, perform physical check-ups, order and interpret diagnostic tests, provide counseling and education, provide treatment, order procedures, refer patients to other health care professionals and specialists, prescribe some medications, and order blood products and oxygen.

Sara Shafiey, NP

Sara is a family health nurse practitioner who has completed her master’s degree in nursing science. Since 2008, Sara has been a member of the registered nurses of Ontario and a member of the Nurse Practitioner’s association since 2012. Sara started working with Jane Finch Family Health Team (JFFHT) in 2013. She, alongside other supporting staff at the JFFHT, helps maintain optimal patient function by promoting patient health through assessment and the provision of supportive, preventive, therapeutic, palliative, and rehabilitative care .

Abi Balasingam, NP

Abi has over 12 years of nursing experience, from acute to primary care setting. In her current role as a certified primary care nurse practitioner, Abi is dedicated to providing care to medically underserved families. Abi is excited to join the interdisciplinary team to continue the work of advancing the quality of health care offered at JFFHT. She is interested in supporting the growth and advancement of NP practice in Ontario. 


 Nurses

Rita Appiah, RN

Rita Appiah is a Registered Nurse that has been with the practice since May 2013. She graduated from the Bachelors of Science in Nursing (BScN) program at York University. Rita is an active member of the College of Nurses of Ontario and the Registered Nurses Association of Ontario. She worked at Brampton Civic Hospital for 4 years in the Clinical Resource Team and has specialized training in pediatrics prior to joining the Jane Finch Family Health Team. As a clinical nurse she is accountable for working with a variety of patients from pediatric to geriatric. She assesses, provides care, and educates patients about their conditions. She works and collaborates with physicians and other healthcare professionals to coordinate the delivery of quality patient care.

Lammi, RPN

Lammi Hassan is a Registered Practical Nurse in good standing with the College of Nurses of Ontario. She is a graduate of Sheridan College’s Accelerated Practical Nursing program, graduating with honours. Lammi is also certificated in venipuncture as recognized by the Regulatory Body of Ontario. With a background in HR and resolution management, her ability to lead others makes her a great addition to the Jane and Finch Family Health Team.

Sophia Kim, RN

Sophia Kim is a Registered Nurse who is an active member of the College of Nurses of Ontario. She graduated from the Bachelors of Science in Nursing(BScN) program at Western University. As a clinical nurse, she is accountable for providing competent and compassionate care to her patients. She has a passion for working collaboratively with physicians and other healthcare professionals to deliver quality patient care and make a positive impact in her community.
